Precede Occupational Health Services Inc. is committed to ensuring the confidentiality of all health records and information and maintains health/employee information in a confidential manner following the Alberta Occupational Health Nurses Association (AOHNA) Confidentiality Guidelines for Occupational Health Nurses and in conjunction with both the Federal and Provincial privacy laws and CARNA.

Confidentiality Policy

All health records/information are maintained in a secure manner and Bonnie L. Krauss is the Privacy Officer for Precede Occupational Health Services Inc. effective July 1, 2005.

All inquiries requiring access and release of information will be addressed by the named privacy officer only.

Precede Occupational Health Services Inc. follows the AOHNA Confidentiality Guidelines for Occupational Health Nurses (1996) requirements. An individual has a right to access any part or all of his/her health record(s); however, written intention to do so must be provided to the privacy officer. All hard copies are maintained at the testing agency.

All health information is stored in locked file cabinets at all times and kept separately from general personnel files. Keys to the locked file cabinets are held by the privacy officer. At this time there is to be no health information stored electronically. Retention laws are in effect of all hard copy information.

Faxing may be used when the original document(s) are not required. Faxing is done in a confidential manner. It is company policy to make arrangements to notify the receiver prior to sending health information. Faxing is only directed to confidential fax lines.

Retention of all health information follows both the federal and provincial retention laws. Audiograms are kept by Provincial Law for 10 years. Pulmonary Lung Function tests are retained for 40 years as per company policy.

Authorized destruction of health information is done by effective shredding or burning as per the direction of the privacy officer.

When the testing agency providing the Occupational Health Service is changed, written attempts to secure consent from the employee(s) will be endeavoured for the transfer of records to the new Health Care Service.
